MIDWEST PULMONARY ASSOCIATES, S.C. is dedicated to providing comprehensive consulting services in pulmonary, critical care and sleep medicine to those patients referred to us by other physicians and patients.  Our physicians are board certified/board eligible in Pulmonary Medicine, Critical Care Medicine and Sleep Medicine.



PULMONARY DISEASE is that subspecialty which deals with anomalies and diseases of the lungs and upper respiratory system.  All of our physicians have achieved expertise and are qualified to diagnose and treat such problems as asthma, emphysema, tuberculosis, pulmonary sarcoidosis, sleep apnea, lung cancer, pneumonia, etc.

 

CRITICAL CARE MEDICINE is that subspecialty which treats the patient who is so seriously ill as to require placement and treatment in the intensive care unit of a hospital where life-threatening illnesses must be treated.

 

SLEEP MEDICINE is that subspecialty which deals with the diagnosis and treatment of sleep disorders.  Polysomnographies (sleep studies) are performed on those patients who are suspected of suffering from such problems as sleep apnea, insomnia, narcolepsy, hypersomnia, etc.  Our physicians are qualified to supervise and interpret such sleep studies.

 

 

IN CASE OF EMERGENCY 

We provide total care for our patients at all times of the day and night; however, if you have a non-urgent problem, we urge you to contact our office and nursing staff during regular business hours.  In the event that you do have an urgent problem, you may call our office number at any time.  If it is at a time other than regular office hours, our answering service will know how to contact the physician on call who will return your call as soon as possible.

In the event you or a member of your family has sudden onset of chest pain, shortness of breath, loss of consciousness, serious accident, etc., you should call 911 and paramedics will give immediate assistance.  Should hospital care be necessary, the emergency room staff of the hospital will contact us for you.

 

TELEPHONE CALLS 

All calls regarding office appointments, prescription refills, billing questions and insurance should be directed to our office during regular office hours.  Messages regarding any of the above will not be taken by our answering service.

 

BILLING POLICY 

We ask that patients pay all applicable copays and deductibles at the time services are rendered.  We will bill all insurances directly for patients having coverage.  HMO patients and any others who require referral authorizations must present referrals upon signing in.  If you do not have a referral for your visit you will need to reschedule your appointment. 

 

MEDICARE 

We participate in the Medicare program and we agree to charge no more than the amount that Medicare approves.  Once a patient has paid the annual Part B deductible amount, Medicare will pay us 80% of the Medicare approved amount directly.  The 20% remaining is the responsibility of the patient.

 

Most patients have some type of supplementary insurance and this coverage may pay most or all of the remaining 20%.  In the case of patients who have supplemental coverage we will file the remaining balance with a copy of the Medicare remittance notice.  Please be aware that if you do not have separate coverage for the 20% Medicare copayment, payment will be expected at the time of your visit.
